Health First Allen County
Health First Allen County (HFAC) is a comprehensive initiative established after legislation designed to better promote and protect Hoosiers’ health was passed in April 2023. That transformative legislation – Senate Enrolled Act 4 – was signed into law by Gov. Eric Holcomb and is modeled on recommendations made in 2022 by the Governor’s Public Health Commission (GPHC).

The grant funding application process was open to projects/programs proposing to address gaps in Allen County for the delivery of identified Core Public Health Services for the 2025 funding award period and in accordance with state law (IC 16-18-2-79.5). Applicants identified one of the Core Public Health Service areas for which to submit an application for the competitive grant funded projects/programs, delineating proposed service delivery programming or improvement processes.

The law establishes Core Public Health Services to be provided by local health departments or via partnerships with community partners.
Our goal is to utilize funding approved by legislation and allocated by county officials to ensure Allen County residents have access to these services to provide optimal health and well-being. This is done through in-house creation of programs and services and/or providing grant funding to provide or bolster programs and services.
The grant funding application process is open to projects proposing to address gaps in Allen County for the delivery of identified Core Public Health Services for the 2025 funding award period only and in accordance with state law.
Applicants may identify one of the Core Public Health Services for which to submit an application delineating proposed service delivery programming or improvement processes. Applicants also can apply for prescribed mini-grants.
