The Allen County Department of Health has designated itself a hybrid entity under the health Information Portability and Accountability Act of 1996 (HIPAA). These are the programs to which this notice applies: Clinical Services, Immunization Services, HIV/STD Prevention, Community Health & Case Management Services, Childhood Lead Poisoning Prevention and Healthy Homes.
