The Allen County Department of Health has designated itself a hybrid entity under the health Information Portability and Accountability Act of 1996 (HIPAA). These are the programs to which this notice applies: Clinical Services, Immunization Services, HIV/STD Prevention, Community Health & Case Management Services, Childhood Lead Poisoning Prevention and Healthy Homes.

The Notice of Privacy Practices describes how medical information about you may be used and disclosed, and how you can get access to the information. Please review it carefully.
