Fatality Review

Fatality review is a collaborative process through which we can better understand why deaths occur. Doing so can help identify ways to prevent future deaths in our community.
Fatality Review Teams in Allen County conduct comprehensive, in-depth reviews of many types of deaths – including circumstances and risk factors involved – and work to understand how and why individuals died, so that future injuries and deaths may be prevented.
Efforts do not assign blame or place judgment. Reviews are neither a criminal investigation nor public record, and meetings of Fatality Review Teams are confidential and not open to the public.
The Allen County Department of Health participates in four Fatality Review Teams:
- Overdose Fatality Review
- Suicide Fatality Review
- Child Fatality Review
- Fetal-Infant Mortality Review

Every person's story can highlight important issues or social challenges. By participating in a Life Story Conversation, you can help Fatality Review Team members gain insight into your loved one's life and experiences. By sharing their story, you can raise awareness of challenges, promote understanding and – more importantly – change.
